Before we can schedule your child for an appointment, we need a few things.  First, you will be asked to complete a few forms.  Next, we ask that you bring us some information including prior evaluations, your child’s educational plan, and pertinent medical records.  All the required forms can be found below.  Please carefully read the document titled Pre-Appointment Information as this provides specific details about was is needed for your child’s file and what to expect at your child’s appointment.  The Pre-Appointment Parent Checklist provides a simple list to ensure that your child’s file is complete.
